Outcome Audit Tool
| This is an assessment tool designed for therapists who wish to audit their outcomes. It is suggested that the therapist administers the questionnaire at the start of the first session and the final session of treatment. By entering the scores in the second core, an outcome graph is produced. |
Clinical Outcomes in Routine Evaluation (CORE) Assessment - This is a 32 point questionnaire that assesses client's feelings of wellbeing, functioning, response to problems and risk of self-harm or harm to others. This can be assessed both at the start and end of therapy. We are committed to auditing outcomes and to that end have devised a simple Excel spreadsheet to enable therapists to obtain immediate visual data on outcomes in the form of a bar chart comparing first and final CORE data with the normative data expressed as mean and SD provided by the system designers at the Psychological Therapies Research Centre, University of Leeds.
All you need to do is enter the number of the response to the CORE questions in the first CORE assessment and for the Second CORE to do the same together with the mean results from the first CORE where it says 'enter previous scores'.
